In the premiere episode of Watson Season 2, all of John's team of doctors are seen reeling with the loss of a neurologist after Ingrid was gone at the end of Season 1. However, John did not want anything common in his team and was looking for someone unusually brilliant like Ingrid.

While interviewing new neurologists, the doctors even run into Ingrid, who tells them she was doing great. She was now going to U Hop and getting a treatment for her personality disorder. The team was unable to find someone with mysteries and secrets like Ingrid, who would make John happy. Since Shinwell got a chance after what happened in Season 1, Ingrid deserved one as well.

In Episode 1 of Watson Season 2, John, with his deductive reasoning, discovered that Elizabeth was not suffering from dementia, but liver failure caused by type 2 diabetes. Mary wanted to donate her liver to her mother, but she was not fit, so John did not want her to proceed and risk the lives of both women.

He then remembered that Elizabeth kept thinking she was pregnant and 25 years old. However, Mary was born when Elizabeth was 29, meaning Mary might have a sibling. They learn that Mary gave birth to a boy and put him up for adoption as she could not afford to take care of him at the time.

Her son, Miles, was a bakery owner, where Elizabeth was a regular customer. However, Miles initially refused to help due to having a family of his own. But when he found out it was one of his regular customers who motivated him to keep doing the job when he wanted to give up, he agreed to doante his liver.

At the end of Episode 1 of Watson Season 2, Elizabeth's case was solved, and she and Mary met Miles's family. Elsewhere, John found none other than Sherlock rummaging through his fridge at night.
